Daycare application checklist

Save this once. Reuse it for every daycare application this season. (Or just open Waitlist Buddy — it does all this for you.)

About your child

  • Full legal name
  • Preferred name / nickname
  • Date of birth (exact)
  • Pronouns (if relevant)
  • Allergies and dietary notes
  • Medical conditions / medications
  • Photo (1 recent, smiling)
  • Birth certificate (digital scan)
  • Immunization record

About you (and your co-parent)

  • Both parents' full legal names
  • Both parents' phone numbers
  • Both parents' email addresses
  • Home address
  • Mailing address (if different)
  • Both employers' names and addresses
  • Emergency contact (one non-parent, with permission)

Care preferences

  • Desired start month
  • Full-time, part-time, or specific days
  • Hours needed (e.g. 8am–5pm)
  • Open to alternate start dates? (very helpful)
  • Languages you'd like child exposed to
  • Pedagogical preferences (Montessori, Reggio, play-based, etc.)

Documents you may need

  • Government-issued ID for parents
  • Proof of address (utility bill)
  • Custody documents (if applicable)
  • Subsidy application reference number (if applying)
  • Doctor's letter (after offer, usually)
  • Immunization record (after offer, usually)

Skip the copy-paste. Let us autofill.

Save this info once in Waitlist Buddy and the Chrome extension will fill it into every daycare form you open.

Tips that save time

  • Make one PDF with birth certificate + immunization record. Reuse it for every centre.
  • Write your "about my child" paragraph once. Edit lightly per centre.
  • Use a single, clear subject line when emailing centres: "Application — [Child Name], born [Month YYYY], [Desired Start Month]". You'd be amazed how much faster you get replies.
  • Don't skip the partner section. Even if you're a solo parent, fill the field with "N/A" rather than leaving it blank — some centres flag empty fields.

FAQ

Can I reuse the same application for multiple daycares?+

The information you provide is usually the same — names, DOB, addresses, preferred start date. The forms differ in layout, which is exactly what Waitlist Buddy autofills.

Do I need a paediatrician letter to apply?+

Not usually for the waitlist itself. You'll often need vaccination records and a doctor's letter once a spot is offered.
